Has your company submitted your OSHA’s 300 form yet? OSHA and the Bureau Labor of Statistics (BLS) requires employers with more than 10 employees to maintain a record of severe illnesses and injuries at work (click here for industry exemptions). The OSHA Form 300 documents reportable injuries and overviews the following information: name and job title of the person who was injured, the number of days off work, and the factors of the incident. According to the United States Department of Labor, covered establishments must submit their Form 300 by March 2nd following the covered year.

Overall, there are three different OSHA 300 Forms that your company may need to submit: the OSHA form 300, 301, 300A. The OSHA 300 form is an incident summary form that provides details regarding what transpired and those involved in a particular injury event, the OSHA 301 form focuses on location and what workplace incidents occurred at that location, and the OSHA 300A summarizes the other two forms and is record of all workplace injuries at all the locations. For larger companies (those with 250 employees or me), they are required to submit all three forms, but for smaller companies (approximately 20-249), only the 300A form is needed.

Anytime a work-related injury results in a death, loss of consciousness, days away from work, restricted work activity/ job transfer, or medical treatment beyond first aid, companies must report this on their OSHA’s 300. If you have questions on how to classify these categories, CLICK HERE. Skin diseases, respiratory conditions, poisoning, hearing loss, and all other occupational illnesses are examples of the classifying illnesses found on the OSHA Form. It is crucial to keep an accurate log detailing all work-related injuries because current and former employees (as well as their representatives) have the liberty to review your company’s OSHA Form 300.
